Sirius Health is committed to protecting the privacy and confidentiality of your personal and health information. We handle your information in accordance with the Privacy Act 1988 (Cth) and the Australian Privacy Principles.
Information we collect
To provide you with safe, high-quality care, we may collect:
- Personal details — your name, date of birth, address and contact details, and an emergency contact.
- Your Medicare number, DVA or healthcare/concession card details, and health-fund details.
- Health information — your medical history, medications, allergies, immunisations, family and social history, and other clinical information relevant to your care.
Wherever practical, we collect this information directly from you. Sometimes we may receive it from others involved in your care — for example a referring doctor, a hospital, or a pathology or imaging provider.
How we use your information
We use your information to provide and coordinate your medical care, to communicate with you (including recalls and reminders — which you can opt out of at any time), for billing and Medicare claiming, and to meet our legal and accreditation obligations. We do not use your information for marketing.
When we may disclose it
We may share your information where it is necessary for your care or where we are permitted or required by law — for example with:
- Other treating practitioners, specialists, hospitals and allied health providers involved in your care;
- Medicare, the Department of Veterans' Affairs, and your health fund for claiming;
- Pathology and diagnostic-imaging providers;
- Accreditation agencies (de-identified where possible); and
- Courts, police or others where disclosure is authorised or required by law.
Storage and security
Your records are held in secure electronic and, where relevant, physical systems, accessible only to authorised Sirius Health staff. Our online bookings are handled through HotDoc, and communication between HotDoc and the practice is encrypted using SSL. Please note that any information you send us electronically (for example by email) could, in transit, be seen by someone other than the intended recipient — so please avoid sending sensitive health details by email.
My Health Record
Where you have a My Health Record, our practice may upload to and access it as part of your care, in accordance with the My Health Records Act 2012. You control your My Health Record and can set your own access and sharing preferences at myhealthrecord.gov.au.
Accessing and correcting your information
You have the right to request access to the personal information we hold about you, and to ask us to correct it if it is inaccurate or out of date. Please contact reception to make a request — we may need to verify your identity, and in limited circumstances allowed by law we may need to decline access and will explain why.
Concerns or complaints
If you have a concern about how your privacy has been handled, please let us know so we can put it right. You can raise it in writing with:

We aim to acknowledge and resolve complaints within 30 days and will let you know the outcome in writing. If you're not satisfied, you can contact the Office of the Australian Information Commissioner (OAIC) at oaic.gov.au or 1300 363 992.
